The Relational Self Therapy

Please contact me by dialing (317) 259-8965 ext. 2 and leave a detailed voice mail. I check my voice mail regularly and try to respond within 24 hours. If this is an emergency, please call 911, your primary care physician, psychiatrist, or go to the local emergency room.
 

Fees:

• Billing rates are per 50-minute therapeutic session
• Phone therapy is available
• Discounted rates are provided for a prepaid minimum of 10 sessions
• Accepted forms of payment include credit cards, check, and cash
• All fees are due at the time of service
• Insurance:
  • In-network provider with Anthem Blue Cross/Blue Shield
  • Out of network claims may be submitted and is the responsibility of the client

Cancellation Policy:
All clients will be billed for any appointment not canceled within 24 hours in advance. If you need to cancel a session, call my voice mail and leave a message with the above-mentioned instructions.
